MSP Fundraising Registration Form

Please complete and submit the information below to begin the process of your fundraising opportunity.

Once you complete this form, someone from MSP will contact you regarding further details and additional information.  You will need to determine whether you will purchase the cards in advance (Option 1) or sell the cards prior to delivery (Option 2).  You will determine the amount of gift cards that you would like to purchase and submit your payment (via credit card) to MSP. 

At the end of your fundraising campaign, return any unused cards to MSP for a refund (minus a nominal restocking service charge of 5%).  Returns must be made within 90 days of the shipping date.
